چکیده

Central pontine myelinolysis (CPM) is a well-known entity first described by Adams et al. in 1959 [1). This demyelinating disorder is associated with electrolyte disturbances (particularly of sodium) and typically occurs in the setting of alcoholism and malnutrition . Extrapontine myelinolysis (EPM) is identified pathologically in about 53% of cases [2] and, of these, thalamic lesions are detected in 34%. Recent case reports have described MR findings in EPM of the putamina, caudate nuclei , and midbrain [3-5). Thalamic lesions were also ident ified in two case reports of CPM [6, 7]. We present three cases in which symmetric thalamic lesions are demonstrated by MR in CPMfEPM and address the characteristic pattern and location of involvement within the thalami.

Dystonia in central pontine myelinolysis without evidence of extrapontine myelinolysis.

A 44-year-old female is described who developed persistent upper extremity and orolingual dystonia several weeks after suspected onset of central pontine myelinolysis (CPM), later confirmed by characteristic pontine lesions on MRI. No foci of the extrapontine myelinolysis were evident. This case confirms that dystonia may be a late and persistent sequela of CPM and may occur in the absence of v...

MR imaging of seven presumed cases of central pontine and extrapontine myelinolysis.

MRI was performed in seven patients with presumed central pontine and extrapontine myelinolysis. The underlying diseases were diabetes, lung cancer, Wilson disease, trauma, alcoholism, renal insufficiency and hemodialysis. CPM was found in four cases (in two of them extrapontine lesions were considered as resulting from Wilson disease), CPM and EPM in three patients. The localization of extrapo...

Resolving MR features in osmotic myelinolysis (central pontine and extrapontine myelinolysis).

Osmotic myelinolysis is a distinctive clinical syndrome with characteristic MR features in the central pons (central pontine myelinolysis) and in other locations (extrapontine myelinolysis). We describe the resolving MR features in an adolescent who has experienced complete neurologic recovery. Regions of involvement manifested increased T2 signal intensity. The extrapontine involvement was not...

Parkinsonism and dystonia in central pontine and extrapontine myelinolysis.

Parkinsonism as well as dystonic signs are rarely seen in central pontine myelinolysis and extrapontine myelinolysis. A 51 year old woman developed central pontine myelinolysis and extrapontine myelinolysis with parkinsonism after severe vomiting which followed alcohol and drug intake, even though marked hyponatraemia had been corrected gradually over six days. Parkinsonism resolved four months...
